Just updated my post on how to fly in and out of Thailand without going through Bangkok with info on direct flights from Chiang Mai to Singapore and Kuala Lumpur. If you are stuck in Bangkok now and need to get out, taking the overnight train to Chiang Mai and then a flight to either of these other regional air hubs might help. Of course, you will need to reorganise your tickets with your airline to fly out of your new exit point.

I saw a Bangkok registered Taxi-Meter in Phuket today. If you stop a cab on the street in Bangkok today and ask to be taken to the Airport the driver will probably offer to take you to U-Tapao, Chiang Mai or Phuket Airport.
